    1. Student Birth Certificate
    2. Student Health Records (immunizations and proof of last physical)
    3. Proof of Residency of Guardian (utility bill, address change notification from the post office, pay stub, rental agreement, purchase contract, etc.)
    4. Proof of Custody (for custodial changes)
